Thanks for the assistance, the only problem I'm having is saving the form in
the proper library. It allows me to save in the Personal Forms library, but
I want everyone on staff to have access to it. To do that I think it must be
saved in the Standard Forms Library. How do I do that?

If you have an Exchange server (I assume you do or you wouldn't be on this
list), you can publish your form to the organizational forms directory where
it will be available to everyone. Start with a plain email message, go to
Tools...Forms...Design this form, and customize it as needed.
