Hold on here. I can't keep everyone's environment in my head. Are you 2003, 2007, 2010, or mixed? I was thinking you were 2003 only.
If you are 2007+ then use exfolders, not pfdavadmin. But yes, I agree that your synopsis is likely. Pfdavadmin comes with a document that describes how to set up a user with the proper permissions. I have the latest version and I get the same result running it on my hub server, a 2003 "utility" server and even an XP box with nothing else on it.
