Are you doing PF replication between servers? Is it possible that even though they aren't mail enabled , replication is using smtp transport to handle this , thus a random email is placed onto the folder? I haven't kept up with all the ins/outs of Exchange over the past couple of years, but thought I would toss it out.

Are you in a position to delete the folder and start again? Sometimes, nuking from orbit is the only way to be sure.
