Nice catch! And which leaves me with a question.
When doing a manual compile, what parameter do I need to add in the
Local/Makefile to ensure that the binary is installed as /usr/sbin/exim4?
I have just realized that it installed /usr/sbin/exim-4.99-2 and created a
symlink for exim to this file. It did not install /usr/sbin/exim4 nor even
create a symlink for it.

In an Internet failure case, the #1 suspect is a constant: DNS.
"Oh, the cruft.", egrep -v '^$|^.*#' ¯\_(ツ)_/¯ :-)
