I see "logpath" set in the [phpmyadmin] section - is it only word wrapping that
has put it after the "#sendmail" comment in this section and others, or is that genuine?
Also, do all other enabled actions appear to be working?

I don't see 'logpath =' set in [phpmyadmin] section.
Take a look at fail2ban.log, it could contain some hints.
