Would it have been pain gradually increasing
over time, worse and worse attacks until finally
he had one so bad he was rolling on the floor in
agony, or could the very first attack have been
that severe, any idea?

Chopra's story is that he collapsed on the floor
right after drinking a glass of orange juice.
Would orange juice have set off an attack that
quickly? Or would it have just been coincidental?
