Yep - looks like it was a caching issue.  From your mention of caching
I also remembered that there is the tool in the admin > COAPI
Management for rebuilding tree metadata.  Rebuilding the categories
tree  helped sort out it out a bit as well.

I think there might be a bit of an issue with the caching when using
the "Manage Keywords" page ie. it doesn't always refresh the cache and
possibly something not entirely right about the way it deletes
branches from the tree.  Using the Category Tree Quick Builder seems
to do a better job of refreshing the tree cache when you add items or
branches.
