One possible issue is that you have a data problem, e.g. duplicate
permission records, one is being edited, the other is being used for
security checks. Another is that your user has multiple roles and there is
some sort of bug where they aren't being resolved properly.

Regarding the disabled buttons - those are meant to imply that the shown
value is inherited. When you click on the button it is toggled to the
opposite of that default. It's very hard to create an intuitive UI for such
a complex relationship, so I'm not surprised that it isn't quite there.
