It's in Elizabeth Zimmerman's Knitting Without Tears.  Knit in garter
stitch, it's a cute and effective jacket--minimal seams of course, the
body is made in one piece, sleeves and hood can be picked up and knit,
so the only seams are the sleeve and the shoulders, and at least the
latter can be grafted, if you know how to graft garter stitch.

Thanks to the garter stitch, it stretches to fit for what seems like
forever :)  5 years, anyway!
