As Steve Fiskum pointed out, you might have to lose "Smart
Extensions" and edit each one by hand if you want to avoid the
problems of extra lines and extensions that are too close to the word.
If this problem has been around since 2006 it probably won't catch
MM's attention, but I'll send in a bug report anyway.
