Additional-

What I just realized... If I open Gnome with my .xinitrc in place  
(exec gnome-panel), the keymap is correct as long as I accept the  
default appearance settings.

The font sizes are pretty small, so I have been going to System >  
Preferences > Appearance to reset them.

I find that all it takes to mess up the keymap is to open the  
Appearance dialog.
