Many AntiVirus products (like TrendMicro's VirusWall or ContentTechnology's
MimeSweeper/MailSweeper) often support  RBL, Anti-Relay (sometimes with
unpleasant twists), set a max size, etc.

So you can elegantly combine the needed features with virus scanning for
mails. Depending on throughput you will need a bigger machine than for a
"pure" mail server though.  And setting up a really secure WinNT is not
really easy.
