Ah well that answers that then. I was expecting it to change the whole background since it shows it in the config. Either way the background color of the characters does not seem to be affected by fish_config. No matter which theme i select it does not change.

Looking at the source code I don't see any support for setting the background color of the entire terminal window. Nor do I see anything in the documentation about it doing so. I'm guessing this is a "TODO" or future enhancement issue.
