Apple did a very nice job in their Human Interface Guidelines of laying
out what's likely to be consistent and therefore intuitive when carried
out in extensions.
Complex programs like ProTools have developed sophisticated ways of
combining mouse input with keyboard accelerators. "Shift-click" to
extend a selection is just the most elementary example.
Most of what's being asked for here can be accommodated by folding
together mouse action with keyboard accelerators.
I do notice that the MSDN docs are pretty mute on the subject.
