Just to clarify a couple of points:

PDFs created with pre-7.0 versions of Acrobat can be re-saved
from Acrobat 7.0 Professional with the Enable for Commenting...
feature turned on.  But it is correct that only files that have been
saved from Acrobat 7.0 Professional can be annotated from 
Adobe Reader 7.0 (the free reader), and only if the feature is
specifically enabled in a separate post-creation step. There is
no way I am aware of to create a commentable PDF directly;
enabling the commenting feature is a post-production operation
that is equivalent to a Save As.

Second, *all* full versions of Acrobat include an enhanced reader
tool that allows users to add notes and markup to PDFs. But the
only version of the free reader that can be used to add notes is
Adobe Reader 7.0, and only if the feature is added to the specific
PDF file by Acrobat 7.0 Professional.

There were some problems with the list last week, and it looks as if
your initial post/s did not go through.  Make sure your list settings
include receiving a copy of your own posts.  If you don't receive your
post back, the chances are that nobody else has received it yet either.

re: #4 on your list: if you are in Acro7 Pro, select 
        Comments > Enable for Commenting and Analysis in Reader
Commenting in Reader is available only on files created using Acrobat 7.
If you are in another version of Acrobat (Acro5/6), there is nothing
special you need to do to the file, but the reviewer will need the
Professional version to be able to comment on the PDF.  They may just
need the Standard version -- I don't remember for sure, and have no way
of checking.
