It's either a failing HDD, or the HDD/controller hardware you're using isn't compatible with FreeBSD. If it's been working for a while without problems, and suddenly started doing this (as you seem to imply) then it's definately failing hardware. If it's been doing this since install, then it'll be just a little harder to debug.
