At any rate IMHO to accomodate the reality that there is already
a large amount of data in electronic format, in a wide variety of
systems with different constraints,  the real solution is to
develop a Centralized Patient ID Service (that's really hard for
a libertarian to admit <g>) where an institution ID is
concatenated with the existing patient ID that they use.  This
will require a significant amount of manual effort. But in the
long run it will reduce/eliminate a lot of work that is being
done now, and reduce the number of mistakes. 

While FreePM is NOT a centralized ID repository, an allowance was
made to help associate foreign data to get it into the proper
EMR. Example:

The list of institutions is configurable through the ZMI and an
unlimited number of ID's can be entered.
