Well I think it is unlikely that only the password will be sent wrong. There is usually so little time between username and password transfers. If the connection has some problems that would effect the username too.
To my knowledge usually this kind of trouble happens when the modem change the DTE speed after connection and the terminal can not adapt. :) But then it shouldnt work the second time user tries.
