I just updated the main home page to include this form, since your

I still have some changes to make, however. Hopefully some JavaScript
to calculate the 5% automatically, for one thing. Eventually, the
home page should be very light on content, and simply point to other
dynamic areas of the site; such as the wiki section, and phpGroupWare
to manage an address book, to-do list, email, inventory and resource
booking, etc. I am working on setting this up...
