Hello Daniel,
if you want to grant smtp traffic per user (out/inbound) you can use
TrendMicro's VirusWall and the eManager pugin, configured as an
e-mail relay for you internal exchange server.
I use this configuration to block smtp traffic for some users that
are allowed only to use e-mail internal. If such a user try to send
outbound e-mail he gets a message that he is not allowed to send
internet e-mail.

The problem is that the SMTP message is not send by the user, it is sent by
the
exchange server so we can't use authentication. :(
