Dennis,
if you have Appleworks you can open it in an Appleworks drawing (or paste it in) and you can fill out the form. The alignment tools etc can make it look pretty spiffy - select required blocks of text and align left.


If you have Keynote that works well too and gives a better looking print. (Need to change page size etc)
