Yes indeed, the Mirrored Drive Door units first came out very noisy. I do
not know all the details, but there have been some fixes with replacement
fans and with software controls over fan speeds when running OS X. If you
seriously consider one of these, it would be best to double check on its
condition.
The original MDD's, known affectionately as the "wind tunnels" did indeed have noise issues. There were two problems, the fans in the power supply produced a constant whine and the main cooling fan was loud. If you booted into OS9 these did a great impersonation of an old vacuum cleaner at full power. There was a noise problem and at first it was not acknowledged as a problem. Apple though stepped up and came out with a power supply/fan replacement program which cured the problem for the majority of users. My Oct 2002 DP MDD sits on a desk less than 3' away and it is now very acceptable. My windooze box makes more noise. All the units built after Mar 2003, and all of the FW 800 MDD's have the improved power supply and fans. For the ultimate in silence the German company Verax introduced their own solution in the form of two kits that made quieter still.

The MDD is a very good Mac, just had some growing pains with the first ones. Supports multiple optical drives, a horde of HD's, 2 GB of RAM and has three PCI slots.
