Have you run DiskWarrior on the Hard Drive? If I had this problem,
that would have been my first choice? I haven't had exactly similar
problems, but it has solved the problems I have had. Just my opinion.

I have a G4/800 iMac Flat Panel running OS 10.3.9. It was doing some
odd things (some items on the desktop had lost their icons, and I had
a buffer under-run while burning an audio book to CD) so I decided to
repair permissions. I run Macaroni for routine maintenance, and was
using it to do the daily, weekly, and monthly tasks and then repair
permissions. The program hangs (in an endless loop I think) on
Permission Repairs.
Any suggestions on fixing this?
