That is definitely a good question. Most of the insanity effects are visual in nature. The screen going all weird, blood showing up on the walls, ceiling, and floor. Seeing things that aren't there. Visions of your self dying. The sounds are bad, but i am told the graphics and special visual effects are way too disturbing for most people.
