Add demo requests page

The demo requests page is still missing. Add it with quite the same
functionality than the previous one, though this little improvement may be
worthwhile:
- the combo box with the demo request names could be hierarchical, reflecting
the directory structure, so the WMS/WCS/WFS requests could be more easily
located. Hiearchy in a combobox can be done with the <optgroup> markup.
