the file system ones (with a index stored in a database)
to get maximum performance (systems ranging to single nation coverage to
whole world 50cm).
Even Paul Ramsey routinely says that storing raster in the db is very good
choice for spatial analysis,
but a poor one to just server imagery. Last time I talked to him he was
looking into improving
that because people insist on storing rasters in a database anyways, and
performance right now
is not on par.

Internal tiling does no good unless you store the internal tiles as
separate reachable
objects in the S3 buckets. You want to have both overviews and inner tiles
quickly
indexable like on a file system (geotiff internal tile directory + mosaic
wide index) to get good speed.
