Review Comment:
   Hmm, what about adding a `ParserState::ColumnDefinition` and using the state 
in `get_next_precedence_default()` to decide if `NOT NULL` should have `Is` 
precedence?  If you're ok with this approach I think it's the cleanest way to 
handle it.
