For websites I tend to use LastPass these days. It's cross
platform, and available on iOS and Android for a small
subscription fee. It makes it extremely easy to use a different,
long, random password on every website, and allows the use of
RFC 6238 two-factor (ie Google Authenticator) to log in to
LastPass itself.

For non-web or more sensitive things, I use KeePass, stashed in a
Dropbox. I suppose for the paranoid you could replace Dropbox
with another syncing mechanism, perhaps BTsync.
