This is caused by staffs that are deleted directly after being
created, eg.
\context Staff { \clef bass; }

They are caused by the spacing algorithm in early lilyponds. The
spacing module was rewritten in 1.2.8; I'd advise you to use a newer
LilyPond, for example one 1.3.52.
