Which Lilypond version do you use?
As long as you have Latin-1 (i.e. ISO-8859-1) characters, it
should work out of the box, at least if you use ly2dvi.
You can write the characters both using their 8-bit
representation (e.g. ``En vänlig grönskas rika dräkt'') or
using the TeX macros, (``En v\"{a}nlig gr\"{o}nskas rika
dr\"{a}kt''). The latter can also be used for some characters
not included in Latin-1. The trick is that ly2dvi uses
\\usepackage[latin1]{inputenc}
which can work together with the low-level font selection
scheme used by Lilypond.
For cyrillic characters or other character sets, you would have
to fool Lilypond to select another font. One possibility could
be to redefine the cmr-alist in lily.scm

I agree that it might be better to use the psnfss font selection
mechanism from LaTeX. However, the reason for the current
solution is that Lilypond calculates the width of each syllable
and must have access to the font metrics file for the specific
font.
