Good question :)  There is some issues returning 404 response codes
instead of 400 response codes - I've filed a bug for that.

I will work on clarrifying the level of support for custom VTIMEZONE
elements in icalendar content and get back to you.

As for this particular issue, I'm not sure why the event succesfully
submits for you but does not for me.  I will take a look at that, but
may need some more information from you (log of HTTP
requests/responses) depending on what I can find on my side.

Were you able to turn on the enhanced logging for this information?  Is
this happening only in production, or also in your development/test
environments?  What percentage of your insert() calls is this occurring
for?
