In IE8 I only see the html of the page as plain text.
In IE8 you can see there's a blank line before the DOCTYPE, so it
would be good to find where in the PHP is creating the white space and
remove it.
In Firefox the MIME type says it's image/png, so you should check for
the header("Content-Type:") in the PHP.
Checking it again just now gives PHP errors before the html:
