This error is definitely an OOPHM-only error, as we keeping the object table
and getting a new connection on refresh are things added in OOPHM.  However,
I haven't been able to reproduce the error so far.  If you have a small
reproduction case that would help.

Also, do you get the same failure in IE or Safari/Mac (you can run them on a
different machine or IE in a Windows VM)?  That would help narrow down where
the problem is.
