I noticed this happening with Etherboot back in 2003. I suspect it's pretty common. Arguably it's a flaw in the DHCP server, which certainly has enough information (the chaddr) to deduce that the two requests are coming from the same interface on the same client. I believe that I worked around the problem using a dhcpd.conf option to say "just use the MAC address as the client key", though I cannot now find this option in the dhcpd.conf documentation.
