I've added the fix for managing the initial and its spacing to the git repository. If you can, install from there. Sorry, but I don't have time to update the Windows installer tonight. Probably won't get to that until this weekend (I've got a paper to write between now and Thursday).

To use the new feature you need the command \setinitalspacing#1#2#3. The three arguments, in order, are the space before the initial, the space reserved for the initial, and the space after the initial. If the space reserved for the initial is 0 then the initial will take up only the space it needs (i.e. the former behavior, which is still the default). All of these distances need to be set in absolute units. If you use a font relative unit (em or ex) then it will be relative to the main document font, not the font of the initial. I'll try to fix this behavior later, but won't have time before the weekend.

This update also fixes a bug I discovered along the way. The spaces before and after the initial, as well as the space between the two annotations will now have the expected scaling behavior when changing the \grefactor.
