Wacky. Try Quartz Debug's Window List (Tools menu/Show Window List).  
That will present you with a list of every process that has one or  
more windows, and if you expand a process in the list, it will show  
you that process's windows.

Measure the size of the unexplained window (using either xScope or  
Pixie and some subtraction), then look for a window of approximately  
that size in the list.
