Hello! So, to give a developer answer to all that is going on here.

First, the double dock icon thing.  As has been said, we cannot reproduce 
this ourselves, and if we can't reproduce the bug, we can't endeavor to fix 
it.  We can make guesses at what might cause it, and what might fix it, but 
they are just that, guesses.  Things like multiple launches or multiple 
copies are possibilities.  

Second, uninstalling.  Growl, like most any other .app application, can 
simply be drug to the trash for a simple removal.  For deleting all 
settings and associated data, like any sandboxed app store application, you 
can delete it via launchpad, and that will remove our "container" where all 
our data is stored along with the app itself (note that this will remove 
ALL registered apps, settings, and notification history)

Third, the start at login thing.  We are sandboxed, and we use the 
officially supported method for programmatically starting our app at login 
under sandboxing.  Sandboxing does not allow access to the methods that 
would add us to the "Start at Login" list in System Preferences, and that 
will not be changing likely, as those API's were horribly insecure.  We 
didn't "invent" anything, these are Apple's approved and documented methods 
for sandboxed apps.
