I've only been using the beta for a week on an XP SP3 machine,, but haven't had any problems like this yet. I did, occasionally in the past. Usually, I could get out of it using the task manager. O Usually, it happened when a program I was working with stopped responding. I could close it, but I had to press the alt e to end the program., Once I got the offending program out of the way, Window-Eyes, usually came back.

The instability was there before the beta. My system crashed much more often before then. I am using Windows 7 and the beta. Now, if it hangs, I can usually close the program I was in and that usually allows me to close and re-open WE. The biggest problem seems to be when switching from one program to another, so you are never quite sure where you are in the list when you alt tab. Sometimes, if I tap the shutdown button, speech is restored, but I have to press enter on my user name login even though I am the only user. At least this way, my open programs keep their status so I don't lose my work.
