I just had a little firewall problem here and don't know if that has anyting to do with it, but I too was finding my start up checkbox set to never, even though I am sure I had it set to always. Also I had an interesting problem where the set options button wouldn't work, that is hitting a space bar or enter on it just didn't activate anything. It's working now correctly, I did do a reboot.

I changed that to: "after logging in, run window-eyes, always". BTW, I used ctrl-u to bring up narrator to fix the problem. A friend suggested trying it.
