Unfortunately it's happening here as well on my XP pro machine.  I know the 
desktop items are on the screen because the mouse keys find them.  The desktop 
won't speak and will actually say "no selected item."  I hit the letters mo and 
pressed enter to see if Mozilla Firefox came up and it did.  So the computer 
has the desktop but WE isn't speaking it when in this state.  It seems to be 
something with Windows Explorer because other Explorer windows exhibit the same 
lack of speech.  I have sent a WE Diag report to GW when WE is in this state.  
Logging off has helped but it seems that a total reboot works better.  Of 
course it's possible that this 3 year old machine has something to do ith the 
problem but it's still frustrating when restarts are necessary instead of 
restarting WE.  If I want to put off the total reboot I'll close WE and run 
NVDA which will still read the desktop.
