What I do if there is 1 file is to tab back and forth and the file
will be read.

Hi Rod,
For me ctrl-shift-s works to read the filename even if there is only one
file in the directory.
But, the arrows speak nothing.
If there is nothing in the folder the ctrl-shift-s reads nothing and I need
to use the mouse to confirm no files.
So, arrows don't do any thing if no files and or if only one file.
Thanks,
Hope this helps.

Go to a folder that contains one file, and try to read it with up/down
arrow keys. Even when "top bottom" index is checked, not only is that
not said, the file name is not said, and the only way I can get
Window-Eyes 9 beta 1 to read anything there is to press f2 to rename the
"file" or "folder.
If more than one the problem is not.
