The problem that I have found with some hotkeys--including Window-eyes and
NVDA is that at times they don't work--unless the desktop has focus. This
is using windows 7 and both USB and PS2 keyboards.

Subject: USB keyboards with window eyes and NVDA.
Hello. I do have two computers, one does not have a usb keyboard it has
a PS2 keyboard, and when I press control alt n, NVDA comes on right
away, and window eyes comes on when I do control alt w. The second
computer it is a Del, and that one I use a USB keyboard, it works fine,
is totally brand new and clean. When I do press control alt n NVDA does
not come on. Same thing happens with control alt w when I want to start
up Window Eyes. Does anyone know why does this happen? I do have
everything installed and all up to date. I even went in to the device
manager and unchecked the box to turn off the feature where you use the
computer to wake it up, and all of the usb route hubs are turned off as
well meaning the power settings from making the computer to save power.
Also when I do assign a different set of hot keys I did try it a few
times, to see if it would work, it does not keep the ability to launch
either NVDA or Window Eyes. Is there a fix to this problem?
