Most late period sleeves tied or buttoned into the armhole. That puffy (shirred) sleeve is attached to the gown, the undersleeve ties under it at the armhole. It is a practical consideration also. A woman could have a "new" gown with the addition of a pair of sleeves!
BTW-- I've done this gown (gown: black wool and sleeves:maroon wool) The sleeves have mega pieces...
