They're not all /that/ successful if spammers really want in either. Launch a 
porn site, require a captcha to show free content, voila your 
so-difficult-to-read-that-even-real-humans-have-trouble-solving-it captcha is 
beaten by crowd-sourcing. Or so I hear.

Something like defensio is pretty good at cutting out spam, even when it 
contains code backtraces and other weird symbols. (Implemented it on 
http://isitruby19.com a while ago, fairly few false positives from it which has 
surprised me. And no false negatives to date.)
