I don't recall any similar problem. What is the block driver and what is the
hardware (e.g. ata_bd in Qemu?) Can you reproduce it with a different 
application? (e.g. bdd, mkxxx)? Did you try the same with a different driver
and/or hardware?

while running grub legacy, I am having troubles with some random behavior 
with disk access. In a single boot I can experience all three kinds of 
situations: 1) working, 2) error -6 (EPERM) during block_read_direct() and 
3) fail during block_init(). These repeat in sets, i.e., a few times it 
works, then suddenly it switches to 2) and repeats and then to 3) for a 
few tries (not necessarily in this order). The second situation also 
causes the app to crash, but I'm not sure if it's the libblock's fault or 
grub's.

Has anyone else experienced this? Any guesses what it might be? Or where 
should I look?
