In our LIS we have one screen that we can customize what appears. We have created a section for quality control and have various topics that only require the user to check the box. We also have a section for free text. Some topics include block mislabeled, slide mislabeled, requisition incomplete, etc. A monthly report is run compiling the data by problem. For issues outside Pathology, a supervisor will talk with the offenders.
