Those gaps, surrounded by spikes in traffic, look a lot like choke, and
I see that your "in" rate is also very high. Try raising your client
"rate" to 100000 or a similarly high number, or raise sv_minrate on the
server to something very high like 100000, and these will likely go away.
Forcing a high rate will hurt players on very slow connections (modems),
but should help just about everyone else. Most clients just do not set a
high-enough "rate".
With NFO, there isn't a concern about the node being overloaded, and an
X5675 is a very fast processor.

You have a X5675 with only 2gb ram? If you are on a vps your host is
probably stuffing the node.

It's happening to everyone on the server, causing players to stutter
with this temporary gap.
The server is a Xeon X5675 @ 3.07GHz, 2gigs ram, running Ubuntu 10.10
Server is 32 players.
This just started happening recently (end of may to now).
I thought it might have been something on the server, I recently
reinstalled the OS and SRCDS. Still occurs with no addons installed.
Replay is disabled.
Any ideas or suggestions?
