-Na, it's not an ATI thing.  I use all Nvidia, and had the same problem a
while back on two different Nvidia-run PCs.  -The same solution fixed it for
me. I remember when I first got this error, it was like no one heard of it
so I just dealt with it.  Then other people started getting it and I found a
working solution (the same one mentioned here) and I made a link for it on
my website: http://genecy.net/Hammer_problem.html

-I thought Valve would have fixed it by now!  Well, at least it's in enough
places to be found easily.

A combination of Vista + Ati card propably caused the blurry vision. 
There was a case about this earlier on forums.
