If so there may be a simple explanation. Mirafone was strongly directed towards the school and college markets. If your horn comes from such source, one of the H-inserts simply may be a misplaced sample from one of teh other school horns. A Swedish friend experienced the opposite situation with a Hoyer single Bb, but got the missing insert, when going back to the selling school. I very much would like getting the photos of your horn.
