Does the printer request you to insert Letter paper? Does the application produce documents for Letter paper? And what exactly do you mean with "make A4 default"? Make A4 default for the printer or for the applications? /etc/papersize is meaningless for the printing system and for alomost all applications (except e.g. groff) because it is is a Debian extension to groff to make it work for Japanese. As soon as the upstream groff has decent support for UTF-8 and Japanese, /etc/papersize will not be needed anymore and may disappear.
